Ten instrumentalny utwór został nagrany jako ilustracja muzyczna filmu Pat Garrett i Billy Kid w lutym 1973 r. w Burbank Studios w Burbank w Kalifornii, w czasie trzeciej sesji. Plonem tej sesji były jeszcze: „Main Title Theme (Billy)”, „Cantina Theme (Workin' for the Law)” (instrumentalny), „Billy” (3 próby), „River Theme” (2 próby, instrumentalny) oraz „Turkey Chase”[1].
"Billy” jest pierwszym wokalno-instrumentalnym utworem na albumie. Jest on uważany przez niektórych krytyków za najlepszą z trzech ballad na tej płycie[2]. Kompozycja ta opisuje zdeterminowanego „desperado”, uciekającego przed stróżem prawa, który kiedyś był jego przyjacielem, i odczuwającego bezwartościowość życia.
Inspiracją dla Dylana mogły być Gunfighter Ballads Marty’ego Robbinsa.
Utwór ten nigdy nie był wykonywany na koncercie.
