The patient was a 45-year-old male who had been in the hospital multiple times with a diagnosis of pseudohypoparathyroidism. He was originally seen with a diagnosis of epilepsy, characterized by seizures which began at the age of 12 with jerking movements, loss of consciousness, tongue biting and incontinence. Serum calcium was 6.5 and phosphate 4.5, with an alkaline phosphatase of 4.2. Parathyroid extract had no effect on his calcium or phosphorus, and calcium chloride induced a change to normal. Biopsy of the parathyroid showed only hyperplastic tissue. For the past nine years he had been off all medication. He had noticed blurring of vision and had cataracts in the right ([1]) and left ([2]) eyes of an anterior radiating pattern. The basal ganglia were calcified. The characteristics of pseudohypoparathyroidism are achondroplasia ([3]), mental deficiency and normal parathyroid biopsy.
